If you are ever unable to make medical decisions for yourself, medical care providers are going to need someone who knows you well to help make medical decisions on your behalf. If you haven’t selected one specific person to be your substitute medical decision maker, your medical team won’t know what’s most important or what kind of treatment or care you prefer.
During this casual, interactive, and fun session, you will learn more about:
- How to carefully select the right substitute medical decision maker for you.
- Tips for talking with that person about what matters most to you
- How having a conversation with your Trusted Medical Decision Maker (TDM) can help your life now
- Making sure others know who you have chosen as your TDM

Presenter Information
John Tastad, MA is the coordinator for Spiritual Support Services at Sharp HospiceCare, and the coordinator for the Advance Care Planning program at Sharp HealthCare.
Now in his 24th year at Sharp, John’s academic training includes undergraduate studies at Bushnell University and graduate studies at Fuller Theological Seminary and the University of San Diego. John is past chair of the San Diego Coalition for Compassionate Care and is actively involved in Bioethics at Sharp and in the community. John has spoken on topics related to Bioethics, Spiritual Care and Advance Care Planning for many national, state and local academic institutions and organizations.
